ELM Cat Rescue Adoption Questionnaire
Every Life Matters Cat Rescue’s primary goal is to ensure the safety, health, and happiness of every feline that we bring into our care. To help us get a sense of each new potential family, we appreciate the time you will take to fill out the following questionnaire. Please answer each and every question truthfully and to the best of your ability.
On occasion, some felines may not be a good fit with a particular family. Because of this, we reserve the right to decline any adoption that is not in the best interests of our rescue cat(s) and/or the potential family.

As of October 15, 2016 our adoption fees are now as follows:
$175 with $50 refunded by EMT upon proof of sterilization
Bonded or Pair of Kittens:
$250 with $50 refunded by EMT upon proof of sterilization of BOTH kittens
Fixed cat or kitten:
Bonded, fixed cats and/or kittens:

***All our rescues have up-to-date vaccinations based on each individual cat or kitten's needs. All cats and kittens are microchipped (if age appropriate). All cats and kittens are tested and treated for parasites found by veterinary, in-house fecal flotation. Also included in the adoption fee is treatment for ear mites, fleas, and/or any other health concerns that need to be addressed prior to adoption. Our adoption fee is an essential part of helping to offset a portion of the costs involved in keeping our rescues healthy and up-to-date with their vaccines and other health care treatment. This token adoption fee is payable upon transfer of the adopted kitten/cat. Payments are accepted via Electronic Money Transfer (EMT), PayPal, or Cash. Deferred payments or partial payments are not accepted. A $50 deposit is required on ALL kittens that are in need of sterilization. The $50 deposit will be returned via EMT upon proof of sterilization.
